Project Summary:
As part of a broader vision to develop an interconnected system in Burnaby within and across sectors, this project aims to promote the health and wellbeing of patients by extending access to community supports that address the social determinants of health. The proposed project will partner with Promatica Digital Ltd., utilizing Social Rx Connect, a proprietary social prescribing software solution that will streamline referral pathways from Family Physicians and Hospitalists to Health Navigators and volunteer Community Connectors, while simultaneously creating channels for community members to access the assistance they require, precisely when it is needed. By exploring avenues to support acute, primary and community care providers to collaborate, this project aims to better empower physicians to address the social, economic, and psychological factors affecting patients' health to improve their health outcomes.
