We need your help to improve the response rate for Vaccine Declarations
As of today, only 72% of employees have submitted their COVID-19 Vaccine Declarations. We need your help to increase this response rate.
The purpose of the declaration is to encourage community members to get vaccinated and provide COVID-19 rapid screening for those who are not vaccinated. It is one of the important layers in place to protect our community this fall.
Starting next week, we will send out rapid screening invitations to those who haven鈥檛 declared their status or who have declared they are not vaccinated.
All declarations are confidential. We take the privacy of our community seriously and have put safeguards in place to restrict access to the information collected. Visit the page to learn more.
Here鈥檚 how you can help employees submit their declarations
- Submit your own Vaccine Declaration if you haven鈥檛 done so.
- Let your teams know how important it is for them to submit their Vaccine Declarations and to do their part to ensure our campuses are safe this fall.
- Provide your team with a link to the website. If they prefer not to use the website, they can request a fillable form by sending an email to covid19@sfu.ca.
- If team members have questions about how their information will be collected and shared, direct them to the information on the Thrive-Health page of the Return to Campus site.
Questions? Email covid19@sfu.ca.
Thank you in advance for your help.
